Yeah this definitely needs attention if it was a normal warp. Was there lag? Did it happen right when Shenk logged in? Were there others where you warped from/to?
 
What's the name of the private warp you were using? Having the name will make it much easier to hunt through the logs. You can PM if you're embarrassed :P
 
/pw goto vault

what i believe happend, is:

at a previous time, i had placed a new row of chests, one of which (im guessing here) was placed on my private warp. so when i warped, the chest was in the way and i got forced down through the map. the floor of the vault is 1 above bedrock. however, i tried it again after i respawned, and i warped into the area underneath the vault floor (there is a little gap there). so im not entirely sure what happend
 
That probably has something to do with it. The game will automatically try to place you on the next legal surface if you warp into a block of any type. I've noticed that I often get stuck inside of blocks when I PW to projects I'm working on, but since suffocation is off, it normally lets me warp out again. You probably ended up with a void kill of some sort.
